These are usually all of the things we need from you on your first visit. Fill the forms online or download and print these out, and take your time before the date of your visit. Of course, if you’d rather have something to do in our waiting room, we’ll have these same forms on hand there as well. Just plan to be there about 15-20 minutes earlier than your appointment time, to make sure you have adequate time to provide this information to our staff.
